New Client Forms
UPDATE: We are now seeing clients exclusively via telehealth in order to protect both clients and staff from COVID-19. As a result, we are no longer using paper forms. We are collecting patient information and sharing documents via our Client Portal.
If you have spoken to your therapist to schedule your first appointment, you should be receiving (or have already received) an email from TherapyPortal inviting you to create a logon and complete some documents. Please complete these as soon as possible. If you have not seen the email, please check your Spam folder; if the email is not there, please let your therapist know right away.

Note: You are not required to pay (co-pays etc.) with a credit/debit card — although nearly all of our clients prefer the convenience of it. However, it is our policy to have a card on file for every client. We will charge your card in accordance with the terms of our Therapist-Patient Services Agreement. If the card we have on file expires or is declined for any reason, you will be required to provide us with a new card.
